How To Use The Dialogue Model

Talking with (referring) professionals and patients/clients the Dialogue Model is helpful to communicate easily in case of multiple, complex health problems on a biological/physical, psychological and social level.

The Dialogue Model can also be used in talks between professionals within the organization and external partners such as education, employement or legal institutions. The input of all participants can be visualized with help of the Dialogue Model framework. It delivers a total overview of which issues are under discussion and how these items can be dealed with in a coherent way.

Multidisciplinary synchronizing and deliberating using the Dialogue Model is particularly helpful in case of discussing ambivalence, in weighing multiple influences, assessment and therapeutic options, in organizing and framing data from different information sources, in striving to achieve equivalent input and using a shared, comprehensive language and if shared decision-making, informed consent, self management and empowerment is wanted.

Dialogically the following emerges:

  • a shared view upon what helps and hinders (possibilities and problems)
  • a joint action plan

Arrangements and aims can well-ordered be record in a visualized, clear care plan that can be evaluated regularly.
